When using Toad and Duo multi-factor login, Toad will require two approvals from Duo before opening your session. It may also try to reconnect a disconnected session using the expired token value, usually as a result of having code insight "pick list" enabled, causing it to attempt to open background sessions to look up field names (i.e., the pick list) for a table.
Managing the Toad sessions
In default configuration, Toad opens two sessions. One is the session you are working in, and the other is a background session that it uses to complete object names when you're typing in the editor. You can choose to retain this functionality and approve the login twice when connecting via Toad, or you can disable the background session. You should also disable code insight pick list.
To disable the background session

Within Toad go to the view menu and select Toad Options

Scroll to Editor, then Display

In the region labeled object name highlighting, remove the tick marks from table names, view names, and stored procedure names.

To disable Code Insight pick list

In Toad Options, go to Editor, then code assist.

Remove the tick mark from Enable Code Insight pick list.
To disable remembering passwords for reconnection
