Request Enterprise Access
Managers or supervisors with employees who have a business need to access databases and enterprise-level applications that use Duo must request access to those services (via an Access Request Form, or ARF).
Note: While it is possible to remove a device from your Duo Device options, it is not possible to remove your account from Duo after opting in.
- Fill out an Access Request Form (ARF) for the application you need access to.
- Managers or supervisors will need to fill out the form on behalf of managed accounts.
- See Add or Remove Device and follow the instructions linked in that resource for how to enroll your device.
- Contact your manager or supervisor for help in filling out any information on the form that you are unsure of.
- You will receive an email confirming the access was granted.
Revoke Enterprise Access
When any University employee (staff, faculty or student employee) or contractor ends their employment with the University, the supervisor needs to revoke access to any Enterprise systems that the employee could access. Complete the following steps to ensure proper revocation of access:
- Complete the Change of Employment Status (COES) form. This form will be automatically routed to Provisioning & Requests, who will notify you once it is complete.
- If the employee had a Duo token, dispose of the token.
