Voice (jacks, phones & voicemail) and Data Jack Request Process
- For your convenience we have provided an optional worksheet (NTS Request Form) which can be attached to your service request. The NTS Request Form is divided into four sections:
- New Telephone Service
- Move/Change Existing Telephone Service
- Data Service
- Other Services needed.
- Submit your request for Voice and Data adds, moves, or changes via ServiceNow.
- Once we have received all information and depending on the complexity of the request please anticipate up to 3 days for an initial response and 2 weeks for the work to be completed.
- To check on the status of a request already submitted please call, 612-625-1355 with your REQ or RITM number.
Existing customers who have access to Service Gateway can use it to manage their own voice and/or data network connections.
