- If you don't already have the form open, begin by finding your form in the form list and click on the title to open the form.
- From the Pages tab, click Add Page.
- You will be presented with two options. Click Create New.
- Note: Do NOT choose Use Existing Template. The U of M is not currently using this functionality. If you use an existing template, you will be required to replace it with a new page prior to moving your form to production.

- After clicking Create New, a pop-up will appear.
- Enter the title of the page. It should begin with your category.
- (Example: T-OIT - Page Name)
- Make sure that Form-Specific is turned on. (This is required.)
- Click Proceed.
- Enter the title of the page. It should begin with your category.
- After clicking Proceed, you will be taken to the Create New Page designer where you can build a new page. Once the page has been created, click Return to Form Structure to return to the form structure page.
- The page will now appear in the Pages section on the left-hand side.
