Conflict of Interest Reporting Forms
Report relationships between University employees and business entities.
Overview
The Report of External Professional Activities (REPA) is an electronic reporting form that captures key details regarding relationships between University employees and business entities. University faculty members and some academic professional and administrative staff (P&A) must file a REPA at least annually.
Filers who hold a "University Official" position also are required to complete a Financial Disclosure for University Officials (FDUO), which will appear after completion of the REPA.
