TDX Approvers: Fill Out a Practice Form

This article is meant to be used for training purposes. You can find your own forms and practice filling them out so that you know what your end user will experience when filling out the form. You can also fill out your own form to practice approving forms for your role.

Finding Your Form(s) in the Client Portal

  1. Go to
  2. Select Client Portal.

    • Enter your U of M Internet ID and password in the appropriate fields and click Sign In. The Client Portal home page opens.
  3. Click Find Access & Services Forms in the Service Catalog. The Service Catalog home page will open.
  4. There are two different ways to find an Access Request Form (ARF) from the Service Catalog home page:
    • Search by alphabetized list:
      1. Click on Services A-Z in the top ribbon.
      2. Click on the Letter that corresponds to the first letter in the name of the ARF form you are looking for (e.g., "D" for Data Warehouse ARF). 
      3. Find the title of the form you want to submit (e.g., Data Warehouse ARF) and click on the name of the form. 
    • Search by Category 
      1. From the Service Catalog home page, scroll down and click on Find all Access Request Forms (ARFs).
      2. Click on the category for the form you are looking for. 
        • Note: There are 8 categories of ARFs depending on the type of access being requested. When you click into a category, it will list all of the forms found within that category.
      3. Click on the title of the form you are looking for. 
  5. Once you have clicked on the title of the form you are looking for, click on the Request Service button on the right side of the page.
  6. Fill out the fields on the access request form. 
  7. Click on the Request button at the bottom of the form to submit the request ticket. A pop-up window will display a message that the request was successfully created.
    • Note the Service Request ID number for training purposes so that you can find your request and work on it in the TDNext side of the tool.

You can select to view the request you just made. Click on View the request you just created from the pop-up window to review your completed request.
